Facility Use Waiver
WAIVER, ASSUMPTION OF RISK, RELEASE OF LIABILITY & MEDIA

AGREEMENT

PLEASE READ CAREFULLY BEFORE SIGNING. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS, INCLUDING
YOUR RIGHT TO SUE.
This agreement applies to all participation in activities and all entry into or use of facilities operated by Wolfhouse MMA
Ltd. and Wolfhouse MMA (collectively, “Wolfhouse”).
1. ACTIVITIES & ASSUMPTION OF RISK
I understand that Wolfhouse provides combat sports, martial arts, fitness and recreational activities including, but not
limited to:
boxing, Muay Thai, kickboxing, Brazilian Jiu-Jitsu, wrestling, grappling, mixed martial arts, sparring, live training, partner
drills, pad work, strength and conditioning, fitness training, open gym, private training, seminars, camps, competitions,
exhibitions and special events.
I understand that these activities involve physical contact and inherent risks of serious injury.
Risks include, without limitation:
 Punches, kicks, knees, elbows, takedowns, throws, submissions and other physical contact;
 Cuts, bruises, sprains, strains and fractures;
 Joint, muscle and ligament injuries;
 Dental, facial and eye injuries;
 Head injuries and concussions;
 Chokes, strangulation and temporary loss of consciousness;
 Neck, back and spinal injuries;
 Permanent disability, paralysis or death;
 Falls, slips, trips and collisions;
 Contact with rings, cages, mats, walls, floors, bags and equipment;
 Equipment malfunction or failure;
 Actions or mistakes of coaches, staff, training partners, opponents, spectators or other participants;
 Participants of different sizes, abilities, strength and experience;
 Fatigue, dehydration, overexertion or medical emergencies;
 Unsafe or imperfect conditions;
 Negligence, acts or omissions of other persons; and
 Other known or unknown, foreseeable or unforeseeable risks associated with participation or use of the facility.
I KNOWINGLY AND VOLUNTARILY ACCEPT AND ASSUME ALL SUCH RISKS, INCLUDING THE RISK OF SERIOUS
INJURY, PERMANENT DISABILITY AND DEATH.
2. RELEASE AND WAIVER OF LIABILITY
In consideration of being permitted to enter Wolfhouse, use its facilities or participate in any activity, I hereby RELEASE,
WAIVE AND FOREVER DISCHARGE:

Wolfhouse MMA Ltd., Wolfhouse MMA, their owners, shareholders, directors, officers, employees, coaches,
instructors, contractors, volunteers, representatives, agents, affiliates, sponsors, landlords, property owners,
event organizers, sanctioning organizations and other participants (collectively, the “Releasees”)
from any and all claims, actions, causes of action, damages, losses, costs, expenses or liabilities arising from or relating
to:
 My presence at Wolfhouse;
 My participation in any activity;
 Use of Wolfhouse facilities or equipment;
 Injury, illness, disability or death;
 Property loss or damage;
 Actions of other participants;
 Equipment or facility conditions; or
 Any other incident connected with Wolfhouse activities.
THIS RELEASE INCLUDES CLAIMS ARISING FROM THE NEGLIGENCE, ACTS OR OMISSIONS OF THE
RELEASEES TO THE FULLEST EXTENT PERMITTED BY LAW.
I agree that I, my estate, heirs, personal representatives and assigns will not bring or maintain a claim against the
Releasees for matters covered by this agreement.
I understand that I am giving up substantial legal rights by signing this agreement.
3. PARTICIPANT RESPONSIBILITY
I am responsible for deciding whether I am physically and medically capable of participating.
I agree to participate within my abilities, follow Wolfhouse rules and follow reasonable instructions given by Wolfhouse
coaches and staff.
I agree to immediately stop participating and inform a coach or staff member if I become injured, feel unwell, experience
symptoms of a concussion or believe that continuing would be unsafe.
Wolfhouse may refuse or discontinue my participation whenever Wolfhouse considers it appropriate for safety, conduct or
operational reasons.
4. MEDICAL EMERGENCIES
If I become injured or experience a medical emergency, I authorize Wolfhouse personnel to contact emergency services
and arrange reasonable emergency assistance when necessary.
I understand that Wolfhouse personnel are not responsible for diagnosing medical conditions or guaranteeing medical
treatment or outcomes.
I am responsible for ambulance, hospital, medical or other costs incurred on my behalf except where otherwise required
by law.
5. PERSONAL PROPERTY
I accept responsibility for my personal belongings.
To the fullest extent permitted by law, Wolfhouse is not responsible for lost, stolen or damaged property, including items
left in lockers, change rooms, vehicles, training areas or common areas.

6. PHOTO, VIDEO, AUDIO & MEDIA RIGHTS
I understand and agree that photography, video recording, livestreaming and other media production may occur
throughout Wolfhouse facilities and during Wolfhouse activities, classes, training sessions and events.
I consent to being photographed, filmed and/or recorded while on Wolfhouse premises or participating in Wolfhouse
activities.
I grant Wolfhouse the unrestricted right to photograph, film, record, reproduce, edit, publish, display, distribute and
otherwise use my name, image, likeness, voice, appearance and performance for lawful purposes including
advertising, marketing, social media, websites, promotional materials, broadcasts, event promotion and other Wolfhouse-
related content.
I understand that I will not be entitled to compensation, royalties or additional approval for such use.
All photographs, videos, recordings and other media content created, commissioned or captured by Wolfhouse,
or by photographers, videographers, employees, contractors or representatives acting for or on behalf of
Wolfhouse, are and shall remain the exclusive property of Wolfhouse, subject to applicable law and any separate
written agreement with the content creator.
I waive any right to inspect or approve the finished content or the manner in which Wolfhouse uses such content.
I release Wolfhouse and its representatives from claims arising from the lawful capture, editing, publication or use of my
image, likeness or voice to the fullest extent permitted by law.
This media authorization continues after my membership or participation with Wolfhouse ends for content lawfully created
while this agreement was in effect.
7. FACILITY RULES
I agree to comply with Wolfhouse rules, policies and reasonable directions from its staff and coaches.
Wolfhouse reserves the right to refuse participation, remove an individual from the facility, suspend privileges or terminate
membership in accordance with its membership terms, policies and applicable law.
8. PARTICIPANTS UNDER THE AGE OF MAJORITY
If the Participant is a minor, the undersigned parent or legal guardian confirms that they have authority to provide the
consents contained in this agreement on behalf of the Participant.
The parent or legal guardian acknowledges the inherent risks of combat sports and martial arts and consents to the
Participant’s participation.
The parent or legal guardian also provides the photography, video, audio and media permissions described in this
agreement with respect to the minor Participant, to the extent permitted by law.
Because the enforceability of liability waivers involving minors may differ from waivers signed by adults, nothing
in this agreement is intended to exclude liability that cannot legally be excluded.
9. GOVERNING LAW & SEVERABILITY
This agreement shall be governed by the laws of the Province of Alberta and the applicable laws of Canada.
If any provision is found invalid or unenforceable, that provision shall be modified or severed to the minimum extent
necessary, and the remaining provisions shall continue in effect.

10. ACKNOWLEDGEMENT
I CONFIRM THAT I HAVE READ AND UNDERSTAND THIS AGREEMENT.
I UNDERSTAND THAT COMBAT SPORTS AND MARTIAL ARTS INVOLVE A RISK OF SERIOUS INJURY,
PERMANENT DISABILITY AND DEATH.
I UNDERSTAND THAT THIS AGREEMENT CONTAINS A RELEASE OF LIABILITY AND THAT I AM GIVING UP
CERTAIN LEGAL RIGHTS.
I sign this agreement freely and voluntarily.
